Physical therapy

Our Physical therapists play important role in helping our patient recover from illness, injuries, and surgery. They will work with patients to help them regain strength, manage pain and discomfort. By working with Physical therapists, patients keep active and prevent/decrease of loss of independence

Our physical therapists will:

Conduct comprehensive evaluations of the client’s home and their environments

Develop individualized care plan and goals tailored to your condition

Conduct an ongoing to determine the patient’s progress and modify treatment plan and goals as needed

Instruct patients and their families on use of assistive equipment or devices

Develop different treatment techniques that help you with movement, reduce pain or discomfort, restore previous function, and prevent any further disability.

Our Private Duty Nurse (PDN) Services

Skilled and unskilled home private duty nursing care (PDN) involves performance of skilled services by a licensed nursing professional (RN/LPN) in the patient’s home, typically to take the place of continued inpatient treatment.

What can a Private Duty/Skilled Nurse do?

Our nurses can observe, manage and treat patients. They take the steps necessary to communicate with the prescribing doctor in order to keep an illness or condition from getting worse. Our compassionate nurses are highly skilled and can help you or a loved one with the following medical needs:

  • Pre-surgery and post-surgery care

Home Health Aide

Home Health Aide’s role:

  • Assist with bathing, shaving and personal care
